About Upperline Health Plus and APP Role (Primary Care Division)
Upperline Health Plus is reshaping value-based primary care with a mission to keep patients mobile, healthy, and out of the hospital. Our model empowers Nurse Practitioners to lead patient care within a highly supported, outcomes-focused environment.
Upperline Health is seeking compassionate and detail-oriented Nurse Practitioners to deliver care within our clinic-based and home-based model, designed to meet the needs of complex adult patients. Working closely with our physician team, APPs develop and manage individualized treatment plans aligned with patient goals, provide diagnostics, deliver education, and coordinate care across multiple disciplines.
Ideal candidates have prior experience in:
- Primary Care / Internal Medicine
- Geriatrics or home-based care
- Palliative or Hospice care
- Value-based care delivery
Most importantly, candidates must be collaborative, mission-driven, and eager to contribute to a rapidly expanding care model that addresses the needs of at-risk patients.
What You’ll Do
- Develop, implement, and manage clinical plans of care for adults with chronic and complex conditions (e.g., diabetes, COPD, CHF, dementia).
- Perform patient assessments via clinic, home, or telehealth visits.
- Educate patients and caregivers on medications, nutrition, disease progression, and crisis prevention.
- Collaborate closely with the interdisciplinary care team to address medical, physical, and psychosocial needs.
- Communicate effectively with Upperline physicians, team members, and community providers to coordinate care.
- Maintain accurate and timely documentation in accordance with state and organizational guidelines.
- Participate in weekly team meetings, case reviews, and scheduled education sessions.
- Provide telephonic clinical support for escalated calls.
- Prescribe medications as permitted by state regulations.

- Monday–Friday schedule, 8 a.m.–5 p.m.
Required Skills & Qualifications
- Master’s degree in Nursing
- Board Certification as Adult NP, Adult-Geri NP, or Family NP
- Current State RN and NP licensure
- BLS certification
- 6+ months NP experience in Primary Care
- DEA registration (or eligibility)
- Prior experience supervising or supporting clinical teams (preferred)
- Reliable automobile, valid driver’s license, and state-required insurance (mileage reimbursed)
- Experience with EMR systems (AthenaHealth strongly preferred)
- Strong communication skills and proficiency with Microsoft Office
