Description
Essential Duties and Responsibilities
Clinical Evaluation & Care Planning:
- Completes comprehensive in-home occupational therapy evaluations and functional assessments.
- Assesses home safety and environmental barriers; recommends modifications and adaptive equipment to support independence and fall prevention.
- Develops and updates individualized, goal-based plans of care in collaboration with patients, caregivers, and the interdisciplinary team.
Treatment & Intervention:
- Delivers occupation-based, task-specific interventions focused on ADLs and IADLs to improve safety, function, and independence.
- Provides training in the safe use of assistive devices and durable medical equipment.
- Implements therapeutic exercise and activity programs and teaches compensatory strategies to support aging in place.citation_8
Patient, Family, and Caregiver Education:
- Educates patients and caregivers on the OT plan of care, safety strategies, and home programs.
- Trains caregivers in safe assistance techniques, body mechanics, and use of equipment.
Interdisciplinary Collaboration & Communication:
- Collaborates with physicians, nurses, and other team members to coordinate care and communicate changes in status.
- Participates in case conferences and team meetings as needed.
Documentation & Compliance:
- Completes timely, accurate, and compliant documentation in the EMR.
- Ensures services meet regulatory, payer, and organizational standards and maintains patient confidentiality.
Professional Practice:
- Practices within state scope and professional standards and maintains current clinical knowledge.
- Provides direction and supervision to Certified Occupational Therapy Assistants (COTAs), as applicable.
Requirements
Education Required:
- Graduate of an accredited occupational therapy program (ACOTE-accredited or successor organization of the American Occupational Therapy Association).
- Master’s or Doctoral degree in Occupational Therapy (preferred).
- Additional coursework or certification in gerontology, home health, low vision, functional cognition, or fall prevention.
Experience Required:
- Minimum of 1 year of clinical experience as an Occupational Therapist (fieldwork experience may be considered).
- Prior home health, geriatric, or rehabilitation experience preferred.
- Experience working with older adults with complex medical conditions (e.g., dementia, stroke, Parkinson’s disease, heart failure, COPD).
