RN Case Manager – Homecare & Hospice
Location: North Conway, NH
Schedule: Fulltime, Monday–Friday, with rotating oncall

About the Role
As an RN Case Manager, you’ll provide compassionate, highquality nursing care to patients in the comfort of their homes. You’ll guide patients and families through recovery, chronic illness management, or endoflife care, ensuring dignity, comfort, and exceptional clinical outcomes.
This is a meaningful, relationshipdriven role where your clinical expertise and emotional intelligence truly matter.
What You’ll Do
Lead patient care through comprehensive assessments, ongoing evaluations, and individualized care planning
Deliver skilled nursing, including wound care, symptom management, medication administration, and patient education
Coordinate services with physicians, therapists, social workers, and home health aides
Support families with education, emotional guidance, and resources
Maintain accurate documentation in the EMR to ensure compliance and continuity of care
Participate in oncall rotation to support urgent patient needs
Promote quality outcomes and reduce avoidable hospitalizations
Qualifications
Active Registered Nurse license in New Hampshire & Maine
Minimum 1–2 years of clinical nursing experience (home care or hospice preferred)
Strong assessment and criticalthinking skills
Reliable transportation and a valid driver’s license
Comfort with mobile documentation and EMR systems

Qualifications
Active Registered Nurse license in New Hampshire & Maine
