At Saving Grace Health, we've built something rare: an in-home medical practice where providers have the time, tools, and support to truly know their patients. We're growing our Kansas City market and looking for an experienced Family Nurse Practitioner who is ready to trade the volume treadmill for something more intentional.

THE ROLE
What you'll do.
- Provide comprehensive care to patients of all age ranges in their homes across the Kansas City metro
- Manage acute and preventive care with the time and autonomy to do it well
- Serve as a consistent, trusted clinical presence for each patient you serve
- Coordinate care with specialists and the broader Saving Grace Health team
- Document care accurately and efficiently using our scheduling and EHR systems
QUALIFICATIONS
What you'll bring.
- Current, active FNP certification (AANP or ANCC)
- Active, unrestricted RN and NP licensure in Missouri and Kansas (or compact license accepted)
- Experience in a concierge, urgent care + primary primary care or similar independent practice setting preferred
- Strong skills in IV placement, assessment, diagnosis, and management across the full family medicine spectrum
- Exceptional interpersonal skills — comfort with home-based, relationship-centered care is essential
- Reliable transportation and the ability to travel throughout the KC metro
- DEA registration or eligibility
SCHEDULE
When you'll work.
- Four (3-4) 12-hour shifts per month (8:00 AM to 8:00 PM) — set days or PRN
- 8:00 AM to 9:00 AM answering phones, triaging/booking patients
- First patient appointment may be booked at 9:00 AM
- Last patient may be scheduled up to 8:00 PM
- On-Call Requirement: Not currently active, but could be activated in future — Two (2) overnight call shifts per month (8:00 PM to 8:00 AM)
- Overnight volume is low
