St. Peters Health Partners is seeking a full time Nurse Practitioner to join our Home Based Primary Care Team, delivering high quality primary care directly to patients in the Eddy Senior Living Communities throughout the Capital Region.
Since 2013, our specialized team has been serving vulnerable individuals who are unableeither temporarily or long termto access care in traditional practice settings. This model provides exceptional care while achieving high patient and caregiver satisfaction, improving health outcomes, reducing avoidable ER visits and hospitalizations, and delaying or preventing nursing home placement. Position Details:
- Full time Home Based Primary Care Nurse Practitioner role providing primary care services directly to senior living patients
- This NP would be providing care in the Eddy Senior Living Communities.
- Home based schedule with start/end from home, no weekends or holidays, and minimal on call
- Patient panels are significantly smaller than a typical primary ca re practice (between 150-185 patients)
- Approximately five scheduled visits per day, allowing time for travel and longer visits due to the complex needs of the patient population

- Our team is co-led by our health systems Continuing Care Executive VP and the Executive Director of our IHANY Accountable Care Organization, providing access to resources and assistance as needed
Requirements:
Applicants should have the following:
- Board Certified Adult Gerontology or Family Nurse Practitioner certification / Internal Medicine
- New York State license and DEA certification
- At least 2-3 years of Nurse Practitioner experience working with an elderly population is required
- Experience working as a Nurse Practitioner in a primary care setting is preferred
Recruitment Package:
St. Peters Health Partners offers a comprehensive salary and compensation package that includes:
- Compensation range: $124,800-$139,360, plus on call pay and mileage reimbursement
- The posted salary reflects the starting range of total compensation and does not include productivity bonuses, incentives related to quality and performance, extra shift incentives, or other forms of cash compensation as applicable to the position
