Position Summary

We are seeking skilled and compassionate Licensed Practical Nurses (LPNs) to join our Long Term Care team. The LPN will work under the supervision of a Registered Nurse (RN) to provide high-quality, patient-centered care and support to residents in a respectful and professional environment.

Key Responsibilities

• Collect and document patient admission information, including medical history and insurance details
• Assist the Registered Nurse in delivering comprehensive nursing care
• Record and monitor vital signs and other health indicators
• Prepare patients for medical examinations and collect test samples
• Administer medications and injections as prescribed
• Treat and dress wounds and bedsores
• Supervise Certified Nursing Assistants (CNAs) as needed
• Support patients with personal hygiene and dressing
• Monitor and document food and fluid intake and output

Qualifications

• Current and valid New York State LPN license
• Up-to-date medical clearance
• BLS certification (Basic Life Support)
• Minimum 1 year of experience in a Long Term Care setting preferred
