Description
American Traveler is seeking a compassionate RN for a Hospice Care Manager position in a home health setting in Vermont, requiring OASIS experience and an active VT or compact RN license.

- Home health and hospice setting, providing care in patients' homes

Requirements
- Active VT or compact RN license currently in hand
- Current BLS/BCLS certification required
- Minimum 1 year of RN experience required
- Home health experience required
- OASIS documentation experience required
- Case management experience required

- Provide compassionate end-of-life care, addressing the physical and emotional needs of patients and their families
- Deliver expert pain and symptom management to hospice patients
- Collaborate with an interdisciplinary team, local physicians, and community providers to optimize patient quality of life
