Responsibilities
Physical Therapists at DHHC screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the patient.
The PT will:
Evaluate the patient's need for physical therapy and develop an appropriate treatment plan. Carry out appropriate treatment programs in accordance with agency policies and professional standards of care.- Communicate with all members of the health care team.
- Document evaluations, treatment goals, and objectives.
- Teach patients and family members the rationale for treatments and procedures.

Qualifications
- Must be a licensed Physical Therapist in Vermont and New Hampshire or hold a compact license.
- Graduate of an accredited school in Physical Therapy
- One year of experience as a PT in a home health care setting is preferred
Required Licensure/Certifications
- Area of Interest: Allied Health
- Pay Range: $39.25/Hr. - $60.84/Hr.
- FTE/Hours per pay period: 1.00 - 1.00 - 40 hrs/week
