We have many openings in Michigan: Lapeer, Davison, Owosso, Saginaw, Charlotte, Grand Ledge, Leslie, Alpena, Caro, Mackinaw City, Mio and Petoskey. Part time Weekends available in Lansing and Jackson. PRN and Part time options. Flexible Monday-Friday reschedules

As a Home Health Physical Therapist:
Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
Observe movements, conduct assessments, document progress and modify care plan as needed
Assist patient with exercises and hands-on therapy to manage pain and increase mobility
Educate patients on their plan of care, established goals, exercises and self-care
Coach family on the plan of care, therapy goals and safely assisting with exercises

We are looking for compassionate physical therapists with:
Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college required
Licensed Physical Therapist issued by the state in which you will be working required
One year healthcare experience
Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
A commitment to consistently meet critical deadlines for charting
The skills needed to self-manage your time and schedule
Demonstrated experience with tablets, mobile phones and EMR software
Schedule and Shift Details Monday-Friday; 8:00a/9:00a-5:00p
Travel
Local travel to residential homes
Requirements
Bachelor's or Master's Degree in Physical Therapy from an accredited college.
Licensed Physical Therapist in state of Michigan. License pending if New Graduate.
