About the role
We are looking for an experienced Physiotherapist to join our home care team in Sydney. As a physiotherapist in this setting, you will play a vital role in maintaining and improving the health, mobility, and independence of older adults. Whilst our support office is based in North Strathfield, this is a predominantly community-based role, with daily travel required to attend in-home aged care clients across Greater Sydney.
Your day will involve travelling to clients’ homes across Greater Sydney, where you will conduct assessments, deliver tailored physiotherapy interventions, and support clients in achieving their mobility and independence goals.
You will spend your time:
- Visiting clients in their home environment
- Conducting initial and ongoing assessments
- Delivering therapy sessions and tailored exercise programs
- Monitoring progress and adjusting treatment plans as needed
- Providing education and guidance to clients and their families
- Completing clinical documentation and planning follow-ups

Key Responsibilities:
- Conduct comprehensive assessments in the client’s home environment, identifying issues and needs
- Design physiotherapy programs tailored to each client’s goals, conditions, and needs. Key focus areas may include mobility improvement, pain management, and prevention of decline
- Deliver therapeutic exercises and mobility training
- Implement evidence-based falls prevention programs, educating clients and caregivers on safe movement and home modifications
- Regularly evaluate progress and adjust treatment plans as needed. Maintain accurate and up to date documentation/clinical notes
- Assess client needs, recommend appropriate assistive technology and equipment and provide training on safe and effective use
Requirements
Essential criteria:
- Bachelor’s or Master’s degree in Physiotherapy and registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Hold a valid Australian Driver's License
- Prepared to travel throughout the Greater Sydney region
- A minimum of 2–3 years of proven clinical experience is required.
- Experience in aged care and/or with NDIS participants is preferred but not mandatory
- Ability to work autonomously with minimal supervision
- Strong command of the English language, with effective written and verbal communication skills.
- Strong organisational skills with the ability to manage time effectively.
