Seven Healthcare are seeking an experienced Occupational Therapist for a travel assignment in New York. This role involves providing case management and direct patient care within a collaborative team environment. Key Responsibilities of the Occupational Therapist
- Participates and assures that Care Delivery Services meet agency goals and customer needs
- Promotes effective working relationships within the team
- Utilizes resources efficiently to contain costs and increase service value
- Promotes positive, respectful communication with patients, families, and external sources
- Develops and revises plans of treatment and discharge plans
- Consults with and educates patients and families about disease processes and self-care techniques
- Provides supervision of COTA and clinical supervision of Home Health Aides
- Ensures completion of all necessary clinical records for compliance
- Participates in patient conferences to ensure coordinated treatment plans
- Promotes quality care through continuous improvement and problem resolution
Requirements of the Occupational Therapist
- Graduate of an approved Occupational Therapy program
- Possession of a current OT license to practice in New York State
- Minimum of one-year experience in a Long Term Care/Home Care setting
- Recent Home Care experience (within the last year)
- Proficient in using OASIS documentation
- Effective communication and interpersonal skills
