✨ Job Summary:
Seeking a dedicated Physical Therapist (PT) to provide rehabilitation services in a skilled nursing and long-term care setting. The PT will focus on improving patient mobility, strength, and overall functional independence through individualized treatment plans.

🔹 Must-Have Skills:
✔ Active Wyoming Physical Therapist License
✔ Strong patient assessment and treatment planning skills
✔ Experience in skilled nursing or long-term care settings
✔ Knowledge of therapeutic exercises and mobility training
✔ Excellent communication and teamwork skills

🔹 Experience Required:
✔ Minimum 1 year of PT experience required
✔ SNF/LTC experience preferred

🔹 Key Responsibilities:
• Evaluate patients and develop personalized treatment plans
• Provide therapy focused on mobility, strength, and functional recovery
• Monitor patient progress and adjust treatment as needed
• Educate patients and caregivers on exercises and rehabilitation goals
• Maintain accurate documentation and compliance with care standards
• Collaborate with interdisciplinary care team
