✨ Job Summary:
Seeking a dedicated Physical Therapist (PT) to provide rehabilitation services in a Skilled Nursing and Long-Term Care setting. The PT will evaluate patients, develop treatment plans, and deliver therapy focused on improving mobility, strength, and overall functional independence.

🔹 Must-Have Skills:
✔ Active Montana Physical Therapist License
✔ Strong patient assessment and treatment planning skills
✔ Experience in Skilled Nursing / Long-Term Care settings
✔ Knowledge of therapeutic exercises and mobility training
✔ Excellent communication and teamwork skills

🔹 Experience Required:
✔ Minimum 3 year of PT experience required
✔ SNF / Long-Term Care experience preferred

🔹 Key Responsibilities:
• Evaluate patients and develop individualized treatment plans
• Provide therapeutic exercise, gait, balance, and mobility training
• Monitor patient progress and adjust treatment plans as needed
• Educate patients and caregivers on therapy techniques and safety
• Maintain accurate and timely documentation
• Collaborate with interdisciplinary rehab team
• Promote patient independence and quality outcomes
