Job Description\n

Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care, developing appropriate plans of care in line with regulatory and clinical practice standards. This position utilizes standardized assessments and evidence-based practices to support clinical interventions and administers various procedures as part of the rehabilitation plan.

\n\nResponsibilities\n

\n Administers manual techniques, ambulation, therapeutic exercises, modalities, and wound care as part of the rehabilitation plan.\n Assesses patient progress and modifies treatments as needed, documenting changes in compliance with regulatory and clinical requirements.\n Evaluates treatment outcomes to ensure effectiveness and adjusts services accordingly.\n Provides consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.\n Communicates effectively with supervisors and healthcare team members about patient progress, barriers, and treatment plans.\n Prepares comprehensive discharge summaries as required by regulations.\n Documents care and maintains patient records in accordance with company policy and all regulatory requirements.\n Participates in required meetings such as patient care conferences and rehabilitation meetings.\n Supervises and supports physical therapist assistants and rehabilitation aides/students as needed.\n Adheres to APTA and state-required practice acts and code of conduct during professional practice.\n\n\nRequired Experience / Certifications / Licensure\n

\n 1 year of Physical Therapist experience is required; new graduates are welcome if they have completed a Level II clinical experience.\n Experience in long-term care or rehabilitation settings is required.\n Current Oklahoma State Physical Therapist License is required.\n\n\nSchedule\n

This position involves 8-hour days with a day shift schedule.
